Application Process
 

To apply for Ontario Works Assistance, call 705-746-8886, ext 500 if you reside in west Parry Sound. If you reside in east Parry sound, call 705-386-2358, ext . 500.

Some general information is gathered by the Ontario Works Office.  The applicant will require the following information when they make first contact with the Ontario Works Office;

  • Social Insurance Number(s)
  • Health Numbers
  • Marital Status
  • Names of All Dependants living with the applicant (You must give the Health Card and Social Insurance Number for all your dependants)

Once the applicants basic information is taken by the Intake Clerk, they will give the applicant an appointment time to come into the office to have their application completed by a Case Worker.  It is very important that the applicant keep their scheduled appointment.  If they miss their appointment, the applicant will have to rebook another time, which will delay approval of benefits.

The following information will be required when the applicant comes in for their appointment;

  • Birth Certificate for the applicant, spouse and dependants
  • Social Insurance Numbers
  • Health Card Numbers
  • Immigration Number and Date of Landing,  if applicable
  • Marital Status
  • Accommodation costs including rent, mortgage, utilities, home/tenant insurance and taxes
  • Address history for the past twelve months
  • Your last three places of Employment
  • Bank Account Information for the last three months
  • Information on earnings and income
  • Court orders/agreements for child/spousal support, if applicable
  • Debts
  • Assets

Application Interview

As part of the application interview, the applicant will be required to attend the following:

  • An employment Information Session
  • An appointment with your Case Worker  to complete a Participation agreement 
  • You may be required to complete a literacy assessment if you have not completed your Grade 12

All mandatory forms will be completed during the application interview including;

  • Application for Assistance, Part 2 to determine the participation requirements and employment activities for the applicant and all adult unit members
  • A Consent to Disclose and Verify Information
  • A Rights and Responsibility Forms*Application for Assistance, Part 1 to determine the financial circumstances or budgetary needs of the applicants
  • A Declaration of Support and Maintenance where applicable
